I undertake to abide by the rules & regulations laid down by Siddh Peeth Shree Lakshminarayan Temple at all times.
I will not misuse my membership in any form of illegal/unethical activity, which may lead to cancellation of my membership.
I will serve the temple in all possible ways to promote the activities of temple that provides for its devotees.
Membership not renewed for 2 consecutive years will be terminated automatically.
All arrears of membership must be paid in full before re-applying for membership.
I here by provide copy of evidence of my Nationality (NRIC/Passport).
